Overall this form has some great features, but I don't see where you could put even simple notes about the contacts. When making contacts I feel it is often a nice feature to simply note any interest, disinterest, or possible suggestions that might be received and useful for future contacts. I do like the set-up of this document and it appears to do some of the work for you in regards to tallying information at the bottom. For some of the business I deal with we track and record our conversations with Government agencies. We note the person contacted, position, and then simple notes. The notes section is fairly important to our business and for that purpose I don't feel I would be able to use this form.

This form could be great "list" and the way it calculates for you in the end is great, but I feel like it needs additional information that is useful when making sales call or contacting potential customers or clients. I could use the form for listing and keeping track of these calls, but think for me I would also have to use another form that allowed me to break down those conversations so I could later review them for follow-up use.

Thanks Brandon. I was looking for a simple, one page document to list my newest leads. Most places on the internet charge an arm and a leg for quality stuff. I really appreciate finding some nice, windows friendly lead documents on your site.

Good form guide for contact list. I think this for is helpful for doing item canvassing jobs.

I like the layout of this form and the ease in which it could be used by many different industries. I could use this form for my sales staff for initiating contact with clients, following up on referrals, or even renewals each month.
